 Adolescents with chronic pain selected from: 1) a representative community sample of 0-18 aged in the Rotterdam area (N=6636) and 2) a random sample from records of 11 general practitioners in the area (N=1456). Selection based on response to question about pain in health questionnaire: 'Did you experience pain in the previous 3 months?. If the response was 'yes' and subjects indicated that the pain lasted longer than 3 months, they were approached for participation in a 2-year follow-up survey. Additional questionnaires were send 6 month later to 12-18 aged who had given consent for follow-up (N=475)

 Mailed questionnaire and diary. Subjects and their parents were sent a pain booklet and additional questionnaires. Adolescents were instructed to keep the pain diary to their own.
