Operationalization
The index consists of nine items measured on a three-point scale of "no", "it depends" and "yes". Each item is assigned a score ranging from zero to two. The possible total scores for the nine items can be from zero (very poor health) to 18 (very good health). To avoid habitual respondents, the statements are phrased both positively and negatively.
